Fall classroom assignments are tentative and likely to change. Room assignments should not be considered final until Saturday, August 24, 2019.
This form should be included, and signed with a physical signature, when requesting that information is sent from Dominican on your behalf.
To request a replacement diploma, the diploma replacement request form must be completed (with a physical signature) and returned to the Office of the Registrar. Please pay the replacement diploma fee through Cashnet and include a copy of your receipt with your request.
